Rescue angioplasty

چکیده
uring the 1980s, thrombolytic therapy became the initial reatment for patients with acute myocardial infarction AMI). Toward the end of the decade, primary percutaneus coronary intervention (PCI) was shown to be effective 1,2), and a recent analysis of 23 randomized trials demontrated improved rates of reinfarction, stroke, and death ompared with thrombolytic therapy (3). However, because f limited facilities and trained interventionists, many hositals continue to initiate thrombolytic therapy for patients ith AMI.

Failure of thrombolysis: experience with a policy of early angiography and rescue angioplasty for electrocardiographic evidence of failed thrombolysis.
OBJECTIVE To assess the outcome of a policy of emergency coronary angiography with or without rescue angioplasty in patients with acute myocardial infarction and ECG evidence of failed reperfusion after thrombolysis. DESIGN A cohort study. SETTING Regional cardiothoracic unit. PATIENTS 197 patients with acute myocardial infarction fulfilling a simple ECG criterion of failed reperfusion. ...
متن کاملEssential thrombocythemia and acute myocardial infarction treated with rescue coronary angioplasty.
A 48-year-old man with essential thrombocythemia suffered an extensive anterior acute myocardial infarction; this is a rare association. A pharmacological thrombolysis was performed, without success. He subsequently underwent successful rescue coronary angioplasty. To our knowledge, there is no other report in the literature relating the triad of essential thrombocythemia, acute myocardial infa...
متن کاملRescue Angioplasty After Failed Thrombolysis: Technical and Clinical Outcomes in a Large Thrombolysis Trial
Objectives. We sought to assess the angiographic outcome, complication rates and clinical features of percutaneous transluminal coronary angioplasty (PTCA) after failed thrombolysis for acute myocardial infarction. Background. “Rescue angioplasty” refers to mechanical reopening of an occluded infarct-related artery (IRA) after failed intravenous thrombolysis. Although the procedure is commonly ...
متن کاملA randomized trial of coronary stenting versus balloon angioplasty as a rescue intervention after failed thrombolysis in patients with acute myocardial infarction.
OBJECTIVES This study was conducted to assess whether coronary stenting produces better results compared with balloon angioplasty in patients with acute myocardial infarction (AMI) after failed thrombolysis. BACKGROUND Little evidence exists on the value of rescue mechanical reperfusion after failed thrombolysis. METHODS This open-label, randomized study enrolled 181 patients with AMI refer...
متن کاملLong-term clinical outcomes after rescue angioplasty are not different from those of successful thrombolysis for acute myocardial infarction.
AIMS The long-term value of rescue percutaneous transluminal coronary angioplasty (PTCA) in patients with ST-segment elevation myocardial infarction who received thrombolytic therapy but failed to achieve early recanalization of the artery is still debated. This study aimed to compare long-term outcomes after successful thrombolysis vs. systematic attempted rescue PTCA. METHODS AND RESULTS A ...
متن کاملRheolytic thrombectomy combined with a protective filter and platelet glycoprotein IIb/IIIa receptor inhibitors in rescue angioplasty.
Rescue angioplasty is used in patients in whom thrombolysis has been unsuccessful, in order to achieve a better restoration of flow and to improve left ventricular function and the long term prognosis. However, patients who undergo rescue angioplasty have a higher rate of reocclusion of the vessel or distal embolism, resulting in increased mortality. Rheolytic thrombectomy, which reduces the qu...
